About this template
Businesses ask contractors, freelancers and vendors for a Form W-9 to get their name, address and taxpayer identification number (TIN). You need it to file information returns, such as Form 1099-NEC. The person you pay fills it in and signs it; you keep it. It isn’t sent to the IRS.
Send it here and they fill in each part online, sign, and you both get the signed PDF. Social Security and employer identification numbers are private fields: they’re in the signed form but hidden on the certificate of completion.
The IRS has its own conditions for accepting Forms W-9 electronically. See “Electronic Submission of Forms W-9” in the IRS Instructions for the Requester of Form W-9.
This is IRS’s official form. Its pages are as published; Dokkiman’s fields take the place of its fill-in boxes. Check IRS’s website for the latest version and instructions.
