About this template
Employees give their employer a Form W-4 so the right amount of federal income tax is withheld from their pay. New employees fill one in when they start, and anyone can give a new one when their situation changes.
Send it here and your employee fills in each step online and signs. You can fill in the “Employers Only” box before sending, or leave it empty. Their Social Security number is a private field: it’s in the signed form but hidden on the certificate of completion.
The IRS sets conditions for employers who take Forms W-4 electronically (see IRS Publication 15). Many states have their own withholding form too.
This is IRS’s official form. Its pages are as published; Dokkiman’s fields take the place of its fill-in boxes. Check IRS’s website for the latest version and instructions.
